Intimation of change.—Every rehabilitation professional shall intimate to the Chief Commissioner, in Form V and within sixty days of the change, any change in the particulars furnished in the application for registration, and the Chief Commissioner shall amend the certificate accordingly on payment of a fee of one hundred rupees.
