Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the rehabilitation professional shall apply to the Chief Commissioner in Form I for a duplicate, accompanied by a fee of one hundred rupees, and the Chief Commissioner shall issue a duplicate certificate marked as such within thirty days of the application.
