Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the gratuity trust shall apply to the Regional Provident Fund Commissioner in Form 5 for a duplicate, accompanied by a fee of one hundred rupees, and the Regional Provident Fund Commissioner shall issue a duplicate certificate marked as such within one hundred and twenty days of the application.
