Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the ambulance operator shall apply to the National Medical Commission in Form 25 for a duplicate, accompanied by a fee of fifty thousand rupees, and the National Medical Commission shall issue a duplicate certificate marked as such within thirty days of the application.
