Application for registration.—Every policy-holder carrying on the holding of a policy shall make an application for registration to the Authority in Form IRDA-4, within fifteen days of the commencement of these regulations, accompanied by a fee of fifty thousand rupees and a statement of every certificate of insurance held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
