Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the payment of sums liable to deduction is carried on, the particulars of every audit report held by the applicant and such other particulars as are specified in Form 3CD, and shall be signed by the applicant or his authorised agent.
