Application for registration.—Every nominee of an insured person carrying on the receipt of benefits as a nominee shall make an application for registration to the Regional Board in Form 2, within thirty days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every benefit held by him on the date of the application, and shall keep a copy of the application and of the statement for twenty-one days from that date.
