Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the nominee of an insured person shall apply to the Regional Board in Form 2 for a duplicate, accompanied by a fee of ten rupees, and the Regional Board shall issue a duplicate certificate marked as such within thirty days of the application.
