Intimation of change.—Every rehabilitation professional shall intimate to the National Trust, in Form I and within one hundred and twenty days of the change, any change in the particulars furnished in the application for registration, and the National Trust shall amend the certificate accordingly on payment of a fee of fifty rupees.
