Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the service provider shall apply to the Consumer Protection Council in Form A for a duplicate, accompanied by a fee of twenty-five thousand rupees, and the Consumer Protection Council shall issue a duplicate certificate marked as such within thirty days of the application.
