Intimation of change.—Every service provider to persons with disabilities shall intimate to the Rehabilitation Council, in Form I and within forty-five days of the change, any change in the particulars furnished in the application for registration, and the Rehabilitation Council shall amend the certificate accordingly on payment of a fee of five hundred rupees.
