Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the service provider to persons with disabilities shall apply to the Rehabilitation Council in Form VII for a duplicate, accompanied by a fee of five hundred rupees, and the Rehabilitation Council shall issue a duplicate certificate marked as such within forty-five days of the application.
