Surrender of certificate.—Every service provider to persons with disabilities who ceases to carry on the provision of services to persons with disabilities shall, within forty-five days of such cessation, surrender the certificate of registration to the Rehabilitation Council in person or by registered post, and shall inform it in Form V of the manner in which every assistive device held by him has been disposed of.
