Application for registration.—Every service provider to persons with disabilities carrying on the provision of services to persons with disabilities shall make an application for registration to the Rehabilitation Council in Form IV, within sixty days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every certificate held by him on the date of the application, and shall keep a copy of the application and of the statement for seven days from that date.
