Application for registration.—Every insurance medical practitioner carrying on the provision of medical benefit shall make an application for registration to the Medical Benefit Council in Form 3, within ten days of the commencement of these rules, accompanied by a fee of fifty rupees and a statement of every benefit held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
