Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the drawal of electricity under open access is carried on, the particulars of every energy bill held by the applicant and such other particulars as are specified in Form VIII, and shall be signed by the applicant or his authorised agent.
