Application for registration.—Every insurance medical practitioner carrying on the provision of medical benefit shall make an application for registration to the Welfare Board in Form 5, within forty-two days of the commencement of these rules, accompanied by a fee of fifty rupees and a statement of every medical certificate held by him on the date of the application, and shall keep a copy of the application and of the statement for thirty days from that date.
