Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the running of a clinical establishment is carried on, the particulars of every certificate of registration held by the applicant and such other particulars as are specified in Form 20, and shall be signed by the applicant or his authorised agent.
