Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the policy-holder shall apply to the Insurance Ombudsman in Form IRDA-3 for a duplicate, accompanied by a fee of two thousand rupees, and the Insurance Ombudsman shall issue a duplicate certificate marked as such within seven days of the application.
