Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the service provider to persons with disabilities shall apply to the State Commissioner in Form III for a duplicate, accompanied by a fee of fifty rupees, and the State Commissioner shall issue a duplicate certificate marked as such within twenty-one days of the application.
