Application for registration.—Every service provider to persons with disabilities carrying on the provision of services to persons with disabilities shall make an application for registration to the State Commissioner in Form VIII, within sixty days of the commencement of these rules, accompanied by a fee of one hundred rupees and a statement of every register of beneficiaries held by him on the date of the application, and shall keep a copy of the application and of the statement for one hundred and twenty days from that date.
