Surrender of certificate.—Every service provider to persons with disabilities who ceases to carry on the provision of services to persons with disabilities shall, within one hundred and twenty days of such cessation, surrender the certificate of registration to the State Commissioner in person or by registered post, and shall inform it in Form II of the manner in which every register held by him has been disposed of.
