Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the employer of a maternity beneficiary shall apply to the Medical Benefit Council in Form 12 for a duplicate, accompanied by a fee of one hundred rupees, and the Medical Benefit Council shall issue a duplicate certificate marked as such within forty-two days of the application.
