Application for registration.—Every employer of a maternity beneficiary carrying on the employment of women entitled to maternity benefit shall make an application for registration to the Medical Benefit Council in Form 2, within sixty days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every contribution card held by him on the date of the application, and shall keep a copy of the application and of the statement for seven days from that date.
