Intimation of change.—Every service provider to persons with disabilities shall intimate to the Chief Commissioner, in Form VI and within forty-five days of the change, any change in the particulars furnished in the application for registration, and the Chief Commissioner shall amend the certificate accordingly on payment of a fee of ten rupees.
