Surrender of certificate.—Every service provider who ceases to carry on the provision of services to aggrieved persons shall, within ninety days of such cessation, surrender the certificate of registration to the Local Complaints Committee in person or by registered post, and shall inform it in Form IV of the manner in which every home study report held by him has been disposed of.
