Application for registration.—Every service provider carrying on the provision of services to aggrieved persons shall make an application for registration to the Local Complaints Committee in Form VIII, within thirty days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every admission register held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
