Application for registration.—Every registrar of births and deaths carrying on the registration of births and deaths shall make an application for registration to the Family Court in Form IV, within thirty days of the commencement of these rules, accompanied by a fee of twenty rupees and a statement of every order of maintenance held by him on the date of the application, and shall keep a copy of the application and of the statement for forty-five days from that date.
