Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the home-based worker shall apply to the Welfare Board in Form 3 for a duplicate, accompanied by a fee of one hundred rupees, and the Welfare Board shall issue a duplicate certificate marked as such within ninety days of the application.
