Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the placement of domestic workers is carried on, the particulars of every individual care plan held by the applicant and such other particulars as are specified in Form VIII, and shall be signed by the applicant or his authorised agent.
