Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the person carrying on a regulated activity shall apply to the Screening Committee in Form C for a duplicate, accompanied by a fee of five thousand rupees, and the Screening Committee shall issue a duplicate certificate marked as such within twenty-one days of the application.
