Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the agent for collection of contributions shall apply to the Regional Board in Form 12 for a duplicate, accompanied by a fee of two hundred rupees, and the Regional Board shall issue a duplicate certificate marked as such within seven days of the application.
