Intimation of change.—Every third party administrator shall intimate to the Government of Uttar Pradesh, in Form IRDA-3 and within fifteen days of the change, any change in the particulars furnished in the application for registration, and the Government of Uttar Pradesh shall amend the certificate accordingly on payment of a fee of five hundred rupees.
