Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the third party administrator shall apply to the Government of Uttar Pradesh in Form IRDA-8 for a duplicate, accompanied by a fee of one lakh rupees, and the Government of Uttar Pradesh shall issue a duplicate certificate marked as such within ninety days of the application.
