Application for registration.—Every third party administrator carrying on the servicing of health insurance policies shall make an application for registration to the Government of Uttar Pradesh in Form IRDA-8, within sixty days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every nomination held by him on the date of the application, and shall keep a copy of the application and of the statement for thirty days from that date.
