Surrender of certificate.—Every third party administrator who ceases to carry on the servicing of health insurance policies shall, within twenty-one days of such cessation, surrender the certificate of registration to the Government of Uttar Pradesh in person or by registered post, and shall inform it in Form IRDA-4 of the manner in which every certificate of insurance held by him has been disposed of.
