Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the consumer shall apply to the Grievance Redressal Forum in Form II for a duplicate, accompanied by a fee of five thousand rupees, and the Grievance Redressal Forum shall issue a duplicate certificate marked as such within thirty days of the application.
