Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the making of donations to a political party is carried on, the particulars of every affidavit of a candidate held by the applicant and such other particulars as are specified in Form 17A, and shall be signed by the applicant or his authorised agent.
