Intimation of change.—Every insurance medical practitioner shall intimate to the appropriate Government, in Form 12 and within sixty days of the change, any change in the particulars furnished in the application for registration, and the appropriate Government shall amend the certificate accordingly on payment of a fee of fifty rupees.
