Application for registration.—Every certifying medical authority carrying on the certification of disability shall make an application for registration to the District Vigilance and Monitoring Committee in Form II, within sixty days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every register of beneficiaries held by him on the date of the application, and shall keep a copy of the application and of the statement for forty-five days from that date.
