Surrender of certificate.—Every certifying medical authority who ceases to carry on the certification of disability shall, within ninety days of such cessation, surrender the certificate of registration to the District Vigilance and Monitoring Committee in person or by registered post, and shall inform it in Form II of the manner in which every register held by him has been disposed of.
