Surrender of certificate.—Every employee who ceases to carry on membership of the Fund shall, within sixty days of such cessation, surrender the certificate of registration to the Controlling Authority in person or by registered post, and shall inform it in Form 5 of the manner in which every claim held by him has been disposed of.
