Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the employee shall apply to the Controlling Authority in Form 5 for a duplicate, accompanied by a fee of one hundred rupees, and the Controlling Authority shall issue a duplicate certificate marked as such within ninety days of the application.
