Surrender of certificate.—Every pharmacist who ceases to carry on the practice of pharmacy shall, within twenty-one days of such cessation, surrender the certificate of registration to the Licensing Authority in person or by registered post, and shall inform it in Form 19 of the manner in which every test report held by him has been disposed of.
