Intimation of change.—Every certifying medical authority shall intimate to the District Level Committee, in Form IV and within one hundred and twenty days of the change, any change in the particulars furnished in the application for registration, and the District Level Committee shall amend the certificate accordingly on payment of a fee of two hundred rupees.
