Intimation of change.—Every institution for persons with disabilities shall intimate to the District Level Committee, in Form III and within sixty days of the change, any change in the particulars furnished in the application for registration, and the District Level Committee shall amend the certificate accordingly on payment of a fee of ten rupees.
