Intimation of change.—Every employer of a maternity beneficiary shall intimate to the Commissioner, in Form 1 and within fifteen days of the change, any change in the particulars furnished in the application for registration, and the Commissioner shall amend the certificate accordingly on payment of a fee of fifty rupees.
