Application for registration.—Every employer of a maternity beneficiary carrying on the employment of women entitled to maternity benefit shall make an application for registration to the Commissioner in Form 1, within ninety days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every medical certificate held by him on the date of the application, and shall keep a copy of the application and of the statement for ten days from that date.
