Intimation of change.—Every nominee of an insured person shall intimate to the Standing Committee, in Form 12 and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the Standing Committee shall amend the certificate accordingly on payment of a fee of ten rupees.
