Forms.—The forms specified in the Schedule shall be used for the purposes for which they are so specified, and where no form is specified for any purpose, an application, return or intimation which contains the particulars these rules require shall be sufficient; Form II and Form IV shall be supplied by the District Vigilance and Monitoring Committee on payment of a fee of fifty rupees, and a form so supplied shall bear the date on which it was supplied and shall be used within seven days of that date.
